First, we have achieved journal status with our periodic publication. From a nomenclature perspective, we will no longer be known as a guide, but more rightly as the comprehensive e-journal we envisioned at inception. Second, by readership demand, we have ceased our print-only existence and migrated to a CD-ROM platform, via Adobe Reader, using secure quarterly email updates. This transition illustrates our collaborative philosophy in the modern health 2.0 participatory era. Of course, all chapter files are printable at your discretion.
